The CAM Lab Account is open to midwives that are current CAM/CALM members.
You will need to fill out this form from the Labcorp site to establish a new account. Be sure to check the box that says "Please have someone contact me about setting up a new account." They will need your name, address, phone number, fax machine number, NPI and license number. Let them know that you want to open a new account that will be linked to the main CAM account. A Labcorp rep from your region will call you back. Please remind the regional rep that we do not need a UPIN since we do not bill Medicare. Not all of them are familiar with the CAM account and discount right away. They may have to find the account and call you back.
If the rep asks you for proof that you are a CAM member, please direct them to this link: Find A Midwife page. The rep can type your name in to verify that you are in the CAM directory. This is the quickest, most efficient way to see who is a current CAM member.
Claudia holds the main account in CAM's name. All other accounts are linked through the main account. The requisition forms will have your name or your practice name listed on them, and below that it will say California Association of Midwives and the PO Box address. Your name and your partners' names are listed to the left. HOWEVER, your billing and your reporting will all go directly to you. Nothing pertaining to your individual account will go to the PO Box, and no one from CAM will have access to any test requisitions/results or billing done through your account.

Pricing:
The prices are based on overall volume and on discounts for frequently ordered tests. There are 2 price lists, Client Special Pricing and Patient Special Pricing.
Client Price: You are the client. This is the price that you pay when the lab bills you directly for tests you order for your "patient." This is the least expensive pricing available to an account. Many midwives use this for their self-pay clients since it is significantly less expensive than having the lab bill the patient directly. The drawback is that you must collect the money from your clients and you are responsible for the lab bill.
Patient Price: This is the price that the lab will charge your client if you have the lab send them a bill directly for their labwork. It is generally 2-5 times more expensive than than Client Pricing. For some reason that I am not clear on, Pap tests can't be client billed so that is why the patient prices take two pages. Midwest Cancer Screening has the best prices on Pap tests with and without HPV-- let me know if you need that information.
There will be a box to check on the requisition if you want LabCorp to bill their insurance.
