The 2009 Licensed Midwife Annual Report

The 2009 Report can only be filled out online. Written clarifications and a link to download the instruction video can be found on the State Statistics page The California Association of Midwives strongly suggests that all midwives either attend one of the trainings or watch the video before submitting your completed form in order to ensure that our statistics are submitted correctly and reflect the quality care given by Licensed Midwives. The online form is due March 31, 2010 and can be found here.
